It has been said that disasters such as airplane crashes are the culmination of 7 decision points where if just one of those decisions had been different, the disaster would have been averted.
Reflecting on this, perhaps just 2 or 3 decision points are leading to our everyday disasters.
However the obvious signs are not always so obvious. Particularly when staff might not know what to look for, staff might not have the time to look for it, staff might be looking for the wrong signs, or staff have become desensitised to the signs they should be heeding.
And of course, staff need to know what to do when they see those signs.
